I purchased this converter about 18 months ago (oct 2002) for my power mac g4, from apple.com, MSRP of about $40, i believe.
Configuration:
- PowerMac Dual G4 1.25ghz MDD
- Two Viewsonic VP181 , from year 1999 and then year 2000. Same exact model, each LCD has a pair of DVI inputs.
- Stock ATI 9000 AGP card
- Additional ATI 7500 PCI card
The ATI 9000 AGP graphics card that came with my Macintosh has both ADC and DVI interfaces. DVI interface works fine with both my flat panels. I bought the Belkin ADC to DVI converter in order to hook up both my Viewsonic VP181's to the same AGP card, so that I could run Quartz Extreme in complete digital.
After receiving it, I tried both monitors through the converter. I have numerous DVI cables, tried every possible configuration, but NO DICE. At first, I thought it was the computer. So I used another exact machine, NO DICE.
Nowhere on the packaging nor on the website did it mention problems similar to mine. Anyway, I google-d around thinking that maybe my LCD's were too old. I found an article of someone with a similar situation:

Summarizing the article, the graphics card people blamed the LCD ppl, and vice versa.
Anyway, I ended up buying that ATI 7500 PCI card and using my 2nd monitor through DVI, but could not use OS X's Quartz Extreme function. This was a costly remedy to the problem.
However, now I am very sure that the Belkin adaptor is at fault, by design. I purchased six months ago the Dr. Bott "DVI Extractor II for ADC". This is basically the same product, except for the fact that Dr. Bott's version WORKS in any configuration I use it in, and the Belkin one does not.
IN SUMMARY:
My LCD's are not to blame, nor are the graphic cards, nor are the two machines. Plain and simple, this Belkin adaptor does not work on some video cards/monitors despite the lack of warnings.
So, don't buy the Belkin version, and go ahead and buy the Dr. Bott version if you have any doubt of whether the adaptor will work with your Macintosh.
PS, I have purchased a Belkin USB hub in the distant past, and that refused to work either. Be forewarned about the firm's bad design.
