The correct idiomatic expression is "As easy as pie ", not "As easy as a pie ". "As easy as an apple pie " is a more-or-less grammatically correct phrase, although not idiomatic.

I am not 100% sure, but I heard question on the standard traditional "American" English.

In the US, there are a lot of dialects, and sometimes, the "American" expressions are differently perceived. I don't remember what type of language EST considers as the standard, but this might cause the big question.

Though this is not a good example, TOEFL accepts the expression, "as easy as an apple pie," but don't accept "as easy as a pie."

The minor thing is that they used to use very American unit measurements such as ounce, which is not globally used. (I believe ml or cc are more common.) When they use American specific unit measurements, non-American people get confused. Moreover, this confusion is caused by something other than English.

Did you ever hear of the controversy surrounding the TOEFL and how some claim it to be biased and/or racist? I see you didn't have any problems like that, but many groups feel "singled out" when they fail the test.
