I have very dry skin. The cold weather and constant washing of my hands after diapering or doing dishes has only increased the cracked and scratchiness of my skin. I've tried Lubriderm, Vaseline and Nivea lotions along with custom creams from body and bath stores.
Aquaphor is like a dream come true. You only need to use a small amount and it feels greasy when first applied. When I tried a sample for the first time it made me question the contents. Aquaphor was absorbed into my skin quickly and the lubrication lasted for hours, even with washing.
It is a little expensive but chain pharmacies offer generics.

Aquaphor is an ointment base containing petroleum jelly and mineral oil. I originally purchased it when looking for something to treat my dry, cracked feet and hangnails.
It's an ointment - not a lotion so it doesn't absorb into your skin quickly. Rather than use it during the day, I rub it into my hands and feet right before bedtime (the jar stays on my bedside table). It has a nice scent and texture.
For the money, this works better than anything else I've tried.
The product is versatile, can be used for diaper rash, minor burns and sunburn, dermatitis, etc. Can be used as a base for making your own ointments (adding calendula, etc.).

Our pediatrician has recommended Aquaphor Ointment for both of our boys, who have slight eczema and dry skin in general. It has wonderful coverage (especially if you rub some between your hands to warm it before applying), and it doesn't seem to sting, even on chapped or broken skin.
I slather both babies with Aquaphor after their baths, and it really helps to keep the natural moisture in their skin from drying out. It's expensive, but it's worth it to keep them from being itchy and uncomfortable. (Note: the pediatrician also prescribed a steroid cream for serious outbreaks; always check with your doctor or pharmacist before following advice online!)
Our only negative about it is that unless you are careful to apply thinly or kind of blot it after applying, it can leave what looks like an oil stain on clothing. It sometimes doesn't come out in the wash, but we haven't worked out whether it's any particular kind of fabric (we mostly use cotton but I have some cotton/spandex T shirts that the stains have stayed on).
